If you are under 18 and starting a job, you may need a work permit. Download the application in your preferred language:

• English Work Permit Application

• Spanish Work Permit Application

• Portuguese Work Permit Application

Then follow these steps:

1. Page 1: Completed by your employer

2. Page 2: Signed by you and a parent/guardian

3. Under age 16: A physician’s signature is also required

4. Bring the completed application to the NHS Main Office
